How to choose input for cic decimation filter?

my input needs to be a DC signal combined with a 5kHz signal. I can't use sine wave from the DSP System Toolbox since it says it can't sample low frequency signals. The signals need to be sampled at at least 2 MHz. Here's what I'm trying to do:
