How to calculate the heat equation in a solid using finite elements?

How can I import a mesh or create a matrix that allows me to solve the attached equation. I attach the physical model that I need to solve, in which are the boundary conditions
I have been able to solve the problem for a flat plate, but for a cylindrical geometry it has been impossible for me to discretize the model.
