How to put row in gui?

I want to add the 4 row name with CAT A, CAT B, CAT C, CAT D. I dont know how to add the four column in gui?

2- I want to do the for the CDFA as the calculation are for only CDFA so if we swich then calculation should be in different way so I want set the condition for that also.

I want to create this table as if you enter the value in the bracket then you will get the value before the bracket value which is addition of altitude ot that area.

See this page in the documentation. Your values get added to the 'Data' property. You can also see some examples in the the uitable documentation. You just need to adapt them for an app designer app.
