how we instantiate signal ports in Simulink ?

Hi. in the attached snap I am trying to use both block output signals to perform XOR operation. but the signal ports are not well instantiate to produce same output as blocks output. how can signal be instantiated in simulink ? so that other block can access the signal ports.
