How do I create an simunik model for the figure attached below

I need to create a simulink block to get an output like below attached figure. To obtain the constantly increasing part, I have used ramp. But I don't know how to add the constant part to it. Any help will be appreciated and Thanks in advance.

Just add the negative slope of the ramp after a certain time. See the attached model for it.
