How to calculate 3D patches of 3D medical CT images data?

I have been calculated 2d patches from medical images. But now I want to calculate 3d patches of the same size from 3d medical CT data. for example, I have a data size of 506*506*300, which is multiple of 23. I want to calculate 3D patches of size (23*23*5), (width*height*depth), please help me. Thanks.....
