How to extract biomarker from MRI scan (.dcm ) image of brain

I am new to MATLAB. I want to extract features from .dcm image(Brain MRI scan). The features should include biomarkers, on the basis of these markers I want to compare images of different patients, with/without Alzheimer's disease. Any sort of guidance will be appreciated. Thanks
