Building a PID controller system

Sorry to ask, but for a project I have to build this PID with the following specifications. I've used MatLab before, but I am not an expert:

English question

build a PID circuit, input is 0 to 5V. Simulate the Vout in Simulink.

Original Chinese question (I am an expat studying in China): PID 類比控制系統實作，在此再結合一PID控制器的實現類比電路，整個系統如下所示， 輸入0->5V 的步階訊號，計算PID 之值，利用Matlab 的Simulink 模擬輸出結果，實際建立以上電路及輸入訊號，利用示波器量出Vo 輸出結果

Questions:

1. Are those breaks or switches? What voltage should I make them close? 2. How can I control the voltage going from 0V-5V? 3. Should I make any corrections on the rest of my already built circuit?
