When using MATLAB Grader in Brightspace D2L, all grades are reported to the same gradebook item

My school uses the LMS integration of MATLAB Grader in Brightspace D2L. I have created 4 different MATLAB Grader questions following the steps below. When testing them, all grades are reported to the same gradebook entry. This causes new grades to overwrite the old grade. How do I get a unique gradebook column for each problem?
Here are the steps I used to create a new problem:
  1. In D2L, go to the course you want to add a MATLAB coding problem to.
  2. Open the Content tool
  3. Add or select a module
  4. Click the Add Existing Activities button
  5. Select External Learning Tools
  6. Click Create New LTI Link
  7. Enter a Name and the URL for MATLAB Grader
  8. Click Create and Insert.
  9. Click Save and display

In D2L, each LTI link ties to a unique gradebook item. You need to create a new LTI link for each problem (step 6 in the list).
Once you have created the first problem, it is tempting to reuse the existing link. This will allow you to create another problem, but reusing the same link is what will tie the problems to the same gradebook item.
Every problem needs to have its own link, which gives each one a unique gradebook item. Give each link a unique, descriptive name, as this is the name that will be used in the Gradebook (unless you use the "Add a grade item..." option). There are instructions at the bottom of this post on how to change the name of an existing link.
(existing links are sorted alphabetically)
The new problem will also appear in the module with this same name as the link, but you can change it if you want. This does not change the name used in the Gradebook.
(Here I changed the name of Problem 2 to Stress-Strain in the module)
The grade items will appear in the gradebook in the order they were created, though they can be reordered. You can also change the name of the item in the gradebook.
(Here I changed the name of the gradebook item for Problem 3 to Kirchoff Loop)
Note that, if you don't use the "Add a grade item..." option, the problems won't appear in the gradebook until the first time a solution is submitted.
In order to change the name of an existing link
  1. Navigate to a Content page
  2. Click the Add Existing Activities button
  3. Select External Learning Tools
  4. At the bottom right of the window, click Manage External Learning Tools hyperlink
  5. Click the expand menu icon next to the link you want to rename and select Edit Link
  6. Enter the new name and click Save and Close
