Simulink RF implement transfer function of a impedance that depends on frequency

I have in my workspace a "Z" tf in s that is the transfer function v/i of an impedance that depends on frequency, I've interpolated some measurements to retrieve it. I need to use it in a simulink RF circuit. I think that I should use S-parameters of RF toolbox to implement the impedance in my circuit but how can I convert tf to s-parameters? And how can I connect it to the rest of the circuit?
