Why do I receive an Error 655 when activating MATLAB?

When activating MATLAB through the activation wizard, I receive an "Error 655: Individual license requires a Username". If I try to manually activate through the License Center on www.mathworks.com, I receive a similar error and no option to input a Username.

This error can occur if an Individual license has had Login Named User enabled, but the license was previously a Designated Computer license.This is a bug. To resolve this, please contact Mathworks Technical Support via the link below:
