thank you very much. so ! when i run any file i see some error ! can you help me how to apply ? how i can extend this with an example if i want to realize any study about FORM.

for example : my limit state function is g(X) = X1+5X2-56

mean_x1 = 23 , std_x1 = 1

mean_x2 = 11 , std_x2 = 1

i want to find :

1. beta (reliability index)
2. probability of faillure
3. design point cordinates

The Software from the Technical University of Munich does return all of these. You can take a look at the examples included in the FORM.zip file on their site to understand the input and output parameters of the included functions, as they seem to be well commented.
