Send only one value from a constant block

Hi, I know this is fairly simple but i somehow stuck at this.
I have this system http://imgur.com/eQ3WL8E
May I ask, how could I make sure only one constant value is sent at a time ?
for example, when t=0, send the first value which is 5, when t=1, send out the second value which is 5.1 etc
