how canI transmit data from SIMULINK model to external custom hardware?

I have to convert the data from DIGITAL TO ANALOG . the digital data is taken from SIMULINK model. DAC is present in external custom hardware bord. how can I transmit the data from SIMULINK model to custom hardware board?
